New Patient Registration

All new patients are required to attend for a pre-registration medical before they are accepted onto the practice’s list, and the receptionist will ask you to book this appointment when you bring in your medical card or fill out the application form.  We also ask that you show us two forms of ID, one photographic and one with proof of address.  If this is not possible please advise the receptionist when you bring in your application form.

We normally register patients to the practice which enables patients to consult with any of the doctors. Nonetheless, patients have a right to express a preference to receive services from a particular doctor and we shall endeavour to comply with any reasonable preference expressed.
